- Monday - Friday, 8 am - 5 pm schedule
- 12 - 15 patients per day
- Three days onsite, two days telehealth
- Home health and clinic-based care
- Medicare-focused geriatric patient population
- Three years adult primary care experience required
- Must be confident working independently
- Telehealth experience required
